The smallpox vaccine scar resulted from the body’s natural healing process. Unlike modern vaccines, the smallpox vaccine was administered using a special two-pronged needle. The person administering the vaccine made multiple punctures in the skin, reaching the dermis layer, which lies below the epidermis.

The vaccine introduced the virus into the body, causing round bumps to form. Over time, these bumps transformed into vesicles—small blisters filled with fluid. Eventually, the vesicles burst and formed scabs, resulting in the distinct scar we see today.
